Annual Reports to the Bishop

Bishop Svennungsen and the synod staff look forward each year to receiving your annual reports to the bishop.  These reports allow us to find out how your ministries are going, your successes and challenges, your insights, and anything that the synod might hold in prayer for you. They also keep us updated on practical information, such as current contact information and anniversaries.

Below you will find links to each of the Rostered Minister Annual Report Forms.

  1. Please open the form that best describes your status over the past year.
  2. Save the form to your computer. Do not fill it out from your browser.
  3. Open the saved form and fill it out.
  4. Be sure to save your changes before sending it to the synod. If you include your last name as part of the file name that would be very helpful.

To use the fillable form, you will need the most current version of Adobe Acrobat Reader. You can download this for free at http://get.adobe.com/reader.  If you use a different PDF reader you may have trouble saving the form, or you may lose all the information when you send the document back.

Once you have completed your Annual Ministry Report, attach it in an email to k.ohm@mpls-synod.org. Alternatively you may print the report and mail it to the synod office (122 W Franklin Ave, Suite 600, Minneapolis, MN  55404-2474).
